Terms and Conditions
About Us
We are Lifestyle Medicine Physicians and are not providing a private GP or acute medical service. We neither refer nor prescribe (with the exception of HRT in our menopause care clinic). When necessary, we strongly recommend the sharing of information with your GP who will offer continuing care.
Pricing
All pricing quoted is valid for 30 days, however, our terms and conditions will remain as stated for the full duration of the provision and/or contract.
Payment Terms
In the case of workplace medicals, payment terms are 14 days from invoice date, unless otherwise stated. Our preferred method of payment is by BACS.
In the case of individual medicals, payment will be taken in advance (in full) by bank transfer or card payment. Please note our cancellation policy below.
Confirmation of acceptance of an appointment confirms that you agree to the terms and conditions here and that you fully understand them.
Cancellation policy
Hexagon Health Limited (HHL) operates a cancellation fee policy
More than 72 hours notice: no charge (other than 2% refund administration fee)
Less than 72 hours notice or failure to attend:
Health Checks: 100% of full cost will apply
Menopause Care Consultations: 100% of full cost will apply
Trucheck: 10% fee to cover cancelled consultation and administrative fee
Blood Test Appointments: 100% of full cost will apply
Extenuating circumstances can be considered on receipt of email to admin@hexagon.health and rebooking the appointment may not incur cancellation fees
It is your responsibility to ensure that you (and/or employees) are informed to arrive at a clinic location 10 minutes before the appointment time. HHL works to a strict appointment schedule and reserves the right to refuse service delivery at full cost to the client company where an individual shows up 10 minutes late to an appointment.
Confidentiality
All medical, financial & Intellectual property related to any contract will remain confidential and will not be divulged to any third party, subject to statutory and legal responsibilities.
Medical information
Retention of all medical records associated with the provision will be held securely by HHL.
Implied understanding and agreement
Bookings made by telephone will be on the understanding that these terms and conditions have been read and agreed.
Laboratory Tests & Costs
Any extra laboratory tests or associated costs that become necessary will be charged to the employer or individual as agreed. This includes repeat tests, or any other associated costs.
Service Commitments for referrals to HHL
Our service standards and commitments to our referral partners are clear and precise; you are working with a company that always delivers high quality medical assessments with timely reporting.
HHL will agree the type of consultation, timing and costs in advance with you the client. Should specialist or GP reports be required following the consultation this will be discussed with you before any request for such a report is made.
When a consultation is arranged with HHL an email with the appointment details will be sent to you, the client.
Our report will be clear, concise and will offer lifestyle-based advice in accordance with a lifestyle prescription.
In the case of employee wellbeing checks the advice will be given to the employee at the time of the consultation. We cannot be held accountable for a breach in confidentiality once the information is given to the employee
In the case of an individual report, we will endeavour to provide a report within 72 hours of the consultation which will be sent via email.
Medical consultations will be conducted in a confidential and ethical manner. The services will comply with the Data Protection Act and Access to Medical Reports Act.
We will be released from all liability to the client if performance of the contract is delayed or prevented by any cause whatsoever beyond our control. I.e. There are unforeseeable circumstances preventing HHL from fulfilling a contract.
